Building Sustainable Physician Retention in FQHCs

One of the biggest challenges facing FQHCs and community-based healthcare organizations today is not simply recruiting physicians; it’s retaining mission-driven leaders who are truly aligned with community health.

It takes a special type of individual to serve underserved communities. In many of my conversations with physicians, the discussion is rarely transactional. Instead, the conversations become heart-to-heart discussions centered around purpose, passion, sacrifice, and the commitment required to care for vulnerable populations.

There is something deeply rewarding about knowing your care has genuinely changed the life of another person. That purpose is what draws many physicians into community health in the first place.

However, once these physicians are recruited, how do we retain them long term?

Yes, community health centers must continue investing in competitive compensation, benefits, retirement planning, and workforce support initiatives to remain competitive. At the same time, physicians entering the community health space must also understand that FQHCs and community-based organizations may not always offer the same resources, infrastructure, or “bells and whistles” found within large health systems.

The solution requires alignment from both sides.

We must continue creating stronger ecosystems around community-based healthcare professionals; environments where physicians feel supported, heard, connected to mission, and empowered to advocate for the communities they serve. Retention improves when professionals feel they are part of something larger than a job description.

As healthcare workforce challenges continue to evolve, I believe the future of retention within FQHCs will depend heavily on culture, mission alignment, leadership support, and creating meaningful platforms for healthcare professionals to make lasting community impact.
